We pulled the published price lists of major private hospitals in all three cities and lined up comparable packages. Kuala Lumpur is the cheapest by a wide margin, Bangkok is closer than most Malaysians assume, and Singapore charges a premium that only makes sense if you already live there.

In brief

  • Entry-level screening (bloods, ECG, chest X-ray, doctor review): about RM330 to RM760 in Malaysia, RM425 to RM670 in Bangkok, RM440 to RM1,860 in Singapore.
  • Bangkok figures are the hospitals’ current promotional prices; list prices are roughly double, so the Bangkok gap can widen when a promotion ends.
  • Executive tier with treadmill stress test, abdominal ultrasound and tumour markers: about RM1,200 to RM1,550 in KL, RM2,300 to RM3,000 in Bangkok, RM5,400 to RM8,500 at Singapore’s Parkway hospitals.
  • Top-end “CEO” programmes: RM2,000 to RM2,800 in Malaysia, RM15,700 in Bangkok, RM12,700 to RM39,300 in Singapore.
  • Like-for-like, Singapore private hospitals charge roughly four to six times Kuala Lumpur. Bangkok charges roughly 1.5 to 2 times.
  • The catch: package names do not travel. A “Premium” in KL is not a “Prestige” in Bangkok. Compare inclusions, not labels.

How we compared

We took prices published on hospital websites and current third-party price trackers between January and August 2026, then converted to ringgit at mid-market rates on 1 September 2026: 1 SGD = RM3.17 and 100 THB = RM12.14. We grouped packages into three tiers based on what they include, not what they are called.

  • Tier 1, Entry: doctor consultation, full blood count, lipid, glucose, liver and kidney function, urinalysis, resting ECG, chest X-ray.
  • Tier 2, Executive: everything in Tier 1 plus a treadmill stress test and/or abdominal ultrasound, plus tumour markers.
  • Tier 3, Comprehensive: Tier 2 plus advanced imaging (echocardiogram, mammogram or bone density, sometimes CT), specialist consultations and add-ons such as body composition or lung function.

Prices are for self-paying patients as published on the date checked. Malaysian and Singapore figures are standard published prices. Both Bangkok hospitals were running promotional pricing at the time of research (Samitivej’s until the end of September 2026), with list prices roughly double the figures shown, so the Bangkok column is a best case. Hospitals change packages often, so treat all of this as a snapshot with the date attached.

At entry level the gap is narrow. Raffles Classic in Singapore is a clinic-based package priced to compete with polyclinics, and Samitivej’s basic check-up is cheaper than Island Hospital’s. Malaysia’s advantage at this tier is mostly Pantai’s RM329 package, which includes a thyroid screen and hepatitis B test that many entry packages elsewhere charge extra for.

Same sources as above. Parkway Shenton packages are delivered at Gleneagles, Mount Elizabeth and Mount Elizabeth Novena hospitals. Bangkok Hospital’s cheaper “Non EST” Prestige variants (THB15,900 and THB22,000) exclude the exercise stress test.

This is the tier most working professionals actually buy, and the tier where the gap opens up. The closest like-for-like match to Pantai KL’s Prestige package (specialist consultation, treadmill stress test, abdominal ultrasound, tumour markers) in Singapore is Parkway’s Screen Signature at S$2,688, about RM8,500, or seven times the KL price. Even Parkway’s Screen Elite, which makes the treadmill and ultrasound optional extras, is 2.5 times Pantai’s all-in price.

Bangkok is the surprise. Bangkok Hospital’s Prestige tier for a man in his 40s is about RM2,300, under twice the KL equivalent, and Thai hospitals bundle age-specific extras (bone density, hormone panels) that Malaysian packages sell separately. For a patient flying in from Jakarta or Dhaka, the Bangkok premium over KL is smaller than the airfare difference in many cases.

At the top end, Malaysia simply does not sell packages at Singapore or Bangkok prices. Island Hospital’s most comprehensive published package is RM2,799. Singapore’s Screen Excelsior costs fourteen times that. Part of this is genuine content (Singapore’s top tiers include CT calcium scoring, MRI options and multiple specialist reviews); part of it is positioning. Malaysian hospitals sell the same advanced scans as individual add-ons. Sunway Medical Centre in KL, for example, lists CT coronary angiography and a comprehensive sleep study as standalone packages rather than folding them into a headline price.

What the price does not tell you

Inclusions vary more than prices do

Two packages at the same price can differ by a treadmill test, a mammogram or five tumour markers. Ask for the full test list before comparing. The name of the tier is marketing.

Turnaround time

Island Hospital offers same-day screening results to international patients, which matters for a traveller on a three-day trip. Singapore private hospitals generally offer a same-day doctor review; comprehensive results follow within days.

Specialist follow-up is where the real cost sits

A screening package is a loss leader. The hospital earns on what it finds. If your screening flags something, the consultation, imaging and procedure prices matter far more than the RM300 you saved on the package. The average inpatient bill for a healthcare traveller in Malaysia in 2025 was RM12,321 (APHM); the equivalent in Singapore is typically several times that.

Currency risk

The ringgit strengthened against the Singapore dollar through 2026, which narrows the gap slightly in ringgit terms. For an Indonesian or Chinese patient paying in their own currency, the KL discount is bigger than the table shows.

Who should not chase the cheapest option

If you have a known cardiac or oncology history, choose the hospital with the specialist you would want to see if something shows up, in whichever city that is. Screening is only useful if the follow-up is fast and in the same building.

The verdict

For a healthy adult buying a preventive executive screening, Kuala Lumpur and Penang offer the best value in Southeast Asia by a margin that survives any reasonable adjustment for inclusions: roughly a quarter of the Singapore price and half to two-thirds of Bangkok. That price gap is the core of Malaysia’s medical tourism pitch, and it is why 1.84 million healthcare travellers came in 2025. The weakness is at the top end, where Malaysian hospitals have not yet packaged and priced a comprehensive product that competes for the executive who is currently flying to Bangkok or Singapore and paying five figures for it. That is a product gap, not a capability gap, and the first Malaysian hospital to close it will own the category.

How much does a full body check-up cost in Kuala Lumpur in 2026?

Published private hospital packages in KL run from about RM330 for a basic screening to RM1,200 to RM1,550 for an executive package with treadmill test and ultrasound, and RM2,000 to RM2,800 for the most comprehensive tiers.

Is health screening cheaper in Malaysia or Singapore?

Malaysia, by a wide margin. Comparable executive packages at Singapore’s Parkway hospitals (Mount Elizabeth, Gleneagles) cost roughly four to seven times the Kuala Lumpur price at September 2026 exchange rates.

Is Bangkok cheaper than Kuala Lumpur for a health check-up?

No. Basic packages are similar, but Bangkok’s executive and comprehensive tiers cost roughly 1.5 to 2 times the KL equivalent, and top-end CEO programmes in Bangkok cost six to eight times more.

Do Malaysian hospitals charge foreigners more for health screening?

Package prices are generally the same for locals and foreigners, but a 6% service tax applies to private healthcare services for non-Malaysians from 2025. Check whether a quoted price includes it.
